Community Living Services

Community Living helps adults with developmental disabilities live in homes throughout the community, with services based on their individual needs.

Program Description

Community Living Services include assistance in finding housing and the provision of individualized supports, as well as other clinical services necessary for adults with disabilities to live independently in their own apartment or home. Adults may choose for themselves which community they will live in, as well as the specific site. Choices include several units owned by Little Friends, some of which offer 24-hour staff on site.

Community Living allows adults with a wide range of disabilities to live in apartments, condominiums and single-family homes as independent members of the community. Individuals maintain their own homes and lifestyles, receiving support and training in their home in areas where help is needed.

Staff assist clients in areas such as independent living skills, community inclusion, and economic self-sufficiency. Clients are encouraged to be as independent as possible and to actively participate in their life choices.

Acting as advocates for the clients in Community Living, staff provide assistance in accessing resources such as Medicaid, food assistance (LINK), and Social Security. Staff also monitor the environmental safety of each home, and coordinate maintenance or repairs as needed. In addition, all inquiries and referrals for services are handled through Support Services.

Licensed or professional staff provide a clinical assessment of each individual’s health status and coordinate services to meet their physical, mental, emotional, and behavioral needs.

Southwest DuPage County in Illinois, including Downers Grove, Westmont, Glen Ellyn, Wheaton, Lisle, far east Aurora, and all of Naperville.

Eligibility

  • Adults, age 21 and older, with a primary diagnosis of a developmental disability
  • Approved for placement and funding by their local Individual Service Coordination agency
  • Approved for funding by the Illinois Department of Human Services
  • In need of supports which Little Friends can reasonably and reliably obtain in the local community
  • The individual chooses to receive support services from Little Friends, and agrees to actively participate in services
  • Not a danger to him/herself or others in the community

Fees

All clients enrolled under Community Integrated Living Arrangements (CILA) will be required to pay a portion of their Social Security or other unearned income toward the cost of services.

FAQ's

  • How can I apply for my family member to receive services from Community Living?

All referrals for our Community Living program must come through a Service Coordination Agency. Referrals from individuals are generally not accepted due to the state requirement of a Pre-Admission Screening.

  • What is a Pre-Admission Screening?

Pre-Admission Screening is an assessment of eligibility for services through the State Medicaid Waiver program. For Little Friends, this is the Community Integrated Living Arrangement (CILA).

  • How do I contact a Service Coordination Agency?
    1. DuPage County residents should call PACT, Inc. at (630) 960-9700
    2. Will County residents (including the Will County portion of Naperville) should contact Service, Inc. at (815) 741-0800.
  • Does Little Friends have a waiting list to receive services from Community Living?

No. Little Friends maintains a database of people who have been referred for services, but a waiting list implies that someone is "next on the list" and that is not the case for our program. Entry into the program is based largely on whether funding is available through the Illinois Department of Human Services. Then when an opening occurs, many additional factors are considered. Currently there are more than 300 people who have been referred to us for services.

  • If state funding is not available, can we consider paying for the services ourselves?

Yes, however the cost ranges from $15,000 - $60,000 per year.
